Peer Support Services
Peer Support Specialists provide a unique perspective for our neighbors. Drawing upon their lived experiences, they will help neighbors navigate through some of those similar challenges. Peer Support Specialists will engage with neighbors when they first arrive at Horizon House, and help them achieve individualized goals that promote addressing mental health, physical health, or substance abuse issues in order to gain financial and housing stability. Peer activities include assertive engagement, modeling positive behaviors/choices, engendering hope, upholding accountability and advocating for equity. Peer Support Specialists will work in coordination with other Horizon House staff including Case Managers and Employment and Guest Service positions as well as with community partners with whom clients are engaged.
Case Management Services
Case Manager engage with neighbors daily to complete both intake assessments and CES (Coordinated Entry System) assessments to help neighbors meet their needs. The intake process ensures a comprehensive assessment to identify appropriate short & long-term interventions. Case Managers also meet with neighbors to make immediate referrals for shelter or housing, medical care, mental health care, legal and vocational services. Additionally, the team works with neighbors to address their need to secure personal identification documents, clothing and transportation.
